Welcome to Madison Oral Surgery & Dental Implants! We’re excited to welcome you to our practice and are here to support you throughout your journey to better oral health. Your first visit is a chance to meet our staff, one of our surgeons, and explore treatment options.

Before you arrive, please take some time to fill out the forms on this page. If you are unable to complete them before arriving, please arrive a few minutes early.

What to Bring

Please bring the following items to your first appointment:

• Referral slip and X-rays, if applicable
• A list of current medications
• Medical/dental insurance information
• Valid identification (e.g., driver's license)
• Form of payment

For Patients Under 18: If you are a minor—under 18 years of age—a parent or legal guardian must accompany you to your consultation appointment.

What to Expect

Before any procedure, you’ll have an initial consultation appointment with one of our oral surgeons. Drs. Lorge, Koob, Hoyer, Streff, Reiland, Walsh, Ebben & Hauge are board-certified oral and maxillofacial surgeons who are dedicated to building relationships, customizing treatment plans, and providing options to ensure you have the best experience possible.

During your first visit, you will receive an oral examination, review health history with your surgeon, and discuss your concerns, goals, and any treatment options you may have researched. Our surgeons will engage in an open conversation to discuss which treatment options are best suited for your needs.

If you require a follow-up appointment for your procedure, our staff will be happy to schedule it and provide you with any relevant information—including detailed instructions—to prepare for your surgery. We look forward to welcoming you and helping you find your best and most beautiful smile!
